老いた私が意識を失う状態、
または意識あっても意思を伝えられない状態となり
自分で身の回りのことができなくなった場合には
以下のようにしてください。
私が自力で飲食できなくなった場合、
無理に飲食させたり、点滴や栄養補給をしないでください。
ましてや鼻管を入れたり
胃瘻を作ったりは絶対しないでください。
私が自分の力で呼吸ができなくなっても
人工呼吸器をつけないでください。
万一、人工呼吸器がつけられている場合でも
一旦は電源を切っていただき、自発呼吸が戻らなかったら
人工呼吸器を取り外してください。
少々意識があっても、場所や日時を言うことができなければ
同様に扱ってください。
そうなったら
昇圧薬も輸血も人工透析も血漿交換などもやめてください。
私が苦しくみえる状態を緩和する治療をしてくださるなら
喜んでお受けします。
ただし、昇圧薬や脳圧低下薬など
延命のための治療はやめてください。
私の命を長らえるために努力してくださる
医療介護スタッフの方々には心から感謝します。
努力してくださる方々には申し訳ありませんが
どうか私の意思を尊重してください。
私はこの意思表明書を意識も清明で書いている内容を
十分理解している状態で書いています。

Statement of intention to end-of-life care (Living Will)
I leave a part of the text of the document
that I wrote to my family and I wrote below.

If I am old, lost consciousness,
or become unaware and unable to communicate,
you can do the following:
If I can't eat or drink on your own,
you shouldn't force me to eat, drink, or feed.
In addition, never put a nasal tube or make a stomach fistula.
Don't wear a ventilator if I can't breathe with my own power.
Even if I have a ventilator attached, turn it off,
and if my spontaneous breathing does not return,
remove the ventilator.
Even if I am a little conscious,
if I can't tell you the place or the date and time,
please treat me as well.
If this happens, stop vasopressors, blood transfusions,
artificial dialysis, and plasma exchange.
If you can treat me to relieve my painful condition,
I will be happy to accept them.
However, refrain from treatments for prolonging life
such as vasopressors and brain pressure-lowering drugs.
I would like to express my heartfelt thanks
to the medical care staff who work hard to prolong my life.
I apologize to those who make an effort,
but please respect my will.
I am writing this statement of intention in a state
where I am fully aware of what
I am writing with awareness and clearness.
